Wow, was this hard to write! Michael gave it to me as a cold lyric, and I went months - maybe a couple years, I'm not sure - trying to write a melody to it.
Michael told me he wanted it to sound a little like Aerosmith's "Dream On". I came up with the guitar motif that I thought did the job perfectly. The verse melody then was fairly easy to write based on the chord progression.
But the chorus - that was hard! I would write one, like it, but then start thinking I could do better. Then one day I decided to include them all! That's why you hear so many variations on the "Do I dare" hook.
Michael did a fantastic job of capturing the experience of depression.
